Implementing universal personal care

In practice, Personalised Care means different things to different people.

As a network of networks, C4PC strives to work with our Partners on what matters most to them and their members.

Through our Partners, we have a hands-on approach to helping organisations to plan for a future in which Personalised Care is engrained in their way of working. We actively work with parts of the system which may inadvertently be preventing Personalised Care from being realised.

Key enablers to embedding the necessary culture change toward Universal Personalised Care include:

  • strong system leadership
  • co-design and co-production at all stages
  • workforce engagement
  • partnership with the (local) Voluntary and Community sector
  • new funding and commissioning frameworks

The Coalition facilitates every part of the system working together with equal voice and influence to achieve our vision. Our network includes people who draw on health and social care services and have experience of voluntary, community and social enterprise organisations, royal colleges, academic bodies, improvement organisations, as well as health and care practitioners from all walks of life. This diversity helps us gather unique insight, share learning, highlight challenges and encourage change.
