This publication describes the aims, outcomes, evidence and lessons learnt for future implementation from the British Heart Foundation (BHF) House of Care programme.
Between 2015 and 2018, BHF funded a £1.5 million programme to offer a new way of delivering person-centred care for people with long-term heart and circulatory conditions (LTCs).
The initiative provided better quality care for people with heart and circulatory LTCs, and had a positive impact for healthcare professionals. Importantly, BHF has added to the evidence base that shows care and support planning can be implemented at practice, community and health system level for multiple long-term conditions.
