Understanding and aligning link worker and community capacity building activity: A place-based approach in York and Wakefield was commissioned by NHS England and delivered by C4PC in collaboration with Community Catalysts, The National Association of Link Workers (NALW), the New NHS Alliance (now The Health Creation Alliance) and the Social Care Institute for Excellence (SCIE).

The Universal Personalised Care (UPC) strategy actions 8 and 9 focus on the recruitment and training of social prescribing link workers and helping build the capacity of communities to support the wellbeing of local people. Other ‘connector’ or ‘link worker’ models are operating in most areas and many organisations and initiatives are concerned with community capacity building in place. This resource focuses on understanding and aligning link workers and community capacity building.
