This report, commissioned by The Richmond Group of Charities Taskforce on Multiple Conditions, identifies a series of key questions and opportunities for change through stories of lived experience and is designed to support the plans and actions of people delivering health and care services alongside others who can make a difference for people living with multiple conditions.
